Phone line offers advanced care planning and palliative care advice
Do any people you care for have pain relief, comfort care or palliative care needs?
Are there times when you don’t know what to do to assist them?
Have you had a case conference with a patient/client and family about advance care planning?
If you answered ‘yes’ to any of these questions, Decision Assist can help you.
General Practitioners and aged care staff around Australia including nurses, allied health professionals and personal care workers can access the latest, expert information and advice about advance care planning and palliative care all over Australia over the phone on 1300 668 908.
Specialist palliative care nurses and doctors give palliative care advice on issues such as medication and symptom management 24-hours a day.
Advance care planning specialists answer how to initiate and support residents, facilitate case conferencing or family meetings and how to complete an advance care plan daily from 8am to 8pm.
The advisory line is part of the broader Decision Assist program, an Australian Government-funded program working to support aged care staff, nurses and GPs nationally. In addition to the phone line, Decision Assist builds linkages in the health and aged care sectors and offers workshops to nurses and GPs in order to improve access to palliative care and advance care planning services for older Australians.
If you, or any of your colleagues have a question regarding advance care planning or palliative care, contact Decision Assist today on 1300 668 908. For more information visit www.decisionassist.org.au.
