We’ve compiled a list of our key patient resources to help patients from diagnosis, through to managing their symptoms. The best thing you can do for your patients is to connect them with Lung Foundation Australia. You can direct them to the living with COPD page or print various resources for use in the clinic to help them feel supported, informed, and confident in managing their condition.
My COPD Checklist
This simple, interactive checklist is designed to help people living with COPD to understand what best-practice care looks like, and what they can do to stay well. This checklist supports self-advocacy, guides conversations in clinical settings, and aligns with the COPD-X Plan and current standards of care.
COPD Online Patient Education (C.O.P.E)
This free online course helps both newly diagnosed patients and people living with COPD to feel more in control of their health. It provides easy to read information, helpful tips and support in six interactive modules.
COPD Action Plan
The COPD Action Plan is to be completed during an appointment with your patient. This action plan supports shared decision-making and helps patients to recognise and respond early to exacerbations.
Respiratory Care Nurse Support Service
Direct your patients to access free phone support designed for people living with COPD from a trained respiratory nurse. Your patient can use the freecall number 1800 654 301, or can book an appointment by completing the form below.
Peer Support Programs and Support Groups
Direct your patients to a range of peer support programs and support groups designed to bring people with similar experiences together to help each other cope with the challenges they may experience.
