COPD Action Plan

The COPD Action Plan should be completed by the clinician and patient together. It guides the patient in recognising when their symptoms change and what action they should take. The COPD Action Plan is available as an editable PDF and in rtf for Medical Software – see instructions below. To be included on our mailing list to receive the COPD update click here. Note: This version of the COPD Action Plan has been developed by the National COPD Coordinating Committee and is currently being piloted by a couple of groups of pulmonary rehabilitation patients. A final version will be available in the near future.
